How to use the invoice template
Use a consistent invoice to make your services, charges and payment details clear. Replace every sample name, number, account detail and date with the correct information before sending it.
The original PDF is a visual template. It does not calculate totals for you.
Your business details
Include your name or business name, address, ABN, email and phone number. Check the invoice heading and GST treatment are appropriate for your registration and the services supplied.
Participant & billing details
Include the participant details required for processing, the invoice number and invoice date. Confirm who receives the invoice, especially when a plan manager processes payment.
Services & charges
The template has space for the NDIS item number, item name, service date, quantity, unit price and line total.
Here is the supplied example with the arithmetic corrected:
| Description | Quantity | Price | Total |
|---|---|---|---|
| Support item and service date | 3 | $60.00 | $180.00 |
| Support item and service date | 5 | $60.00 | $300.00 |
| Total AUD | $480.00 |
Template correction: The original Canva document and PDF display $580.00. The two example line items add to $480.00. The invoice download on this page has been corrected while keeping the original layout. These example prices are not a current NDIS rate schedule.
Payment information
Record the correct account name, BSB and account number, or other agreed payment instructions. State the agreed payment due date. Remove the sample payment details from the original template.
